Team-Based Care and Support

Our team’s sole purpose is to service DCC member practices:

  • Practice Liaison
    • Provides guidance during practice transformation initiative
    • Helps practice meet annual goals (well-visits, care management for diabetes, heart failure, COPD)
    • Evaluates all patient processes to increase efficiency, help define roles of staff and capture key metrics
    • Helps practice realize missed revenue opportunities
  • Care Management Team
    • RN assigned to your practice to address high-risk patients with multiple chronic illnesses
    • Social Worker to address social and behavioral barriers to care and link patients to appropriate programs and services
    • Offers on-site and in-home services for patients to help identify social and/or behavioral reasons for lack of treatment/RX compliance and difficulties keeping medical appointments
  • Data Analytics
    • Creates custom reports to help meet your practice goals (year/year well visit reports, identify populations by diagnosis, etc.)
